Bronchodilator testing: an endless story.

چکیده
T he history of the bronchodilator test dates back to the late 1950s when airway diseases were classified as reversible or irreversible depending on the response to bronchodilators [1, 2]. The hope to differentiate asthma from chronic obstructive pulmonary disease (COPD) on this basis was such that the test soon gained popularity in clinical practice. Two large trials published in the mid 1980s, however, dimmed the original enthusiasm on such a test because no clear cut-off was found between asthmatic and COPD patients [3, 4]. In addition, the response depended on several factors, such as definition and severity of airflow obstruction and spirometric parameter considered for the functional response to the dilator agent together with its relevant threshold of natural variability. Finally, the test was scarcely reproducible over time in the individual patients. Somehow, these two studies challenged the common notion of that time that airway reversibility was a typical and unique feature of bronchial asthma.

Bronchodilator response in patients with normal baseline spirometry.
BACKGROUND Spirometry before and after bronchodilator is performed to assess air flow-limitation reversibility. In patients with normal baseline spirometry the frequency of a positive bronchodilator response, as defined by American Thoracic Society/European Respiratory Society criteria, has not been described. METHODS We retrospectively analyzed adult patients tested in 2 academic pulmonary ...
متن کاملAnother Mary
There are story book Marys including "Mary, Mary, quite contrary"; historical Marys, including the Magdalenes and queens, Marys of the legitimate and illegitimate stage?an endless procession of them, sometimes masquerading as Mary Annes, Mary Elizabeths, or Mary Louises. It was one of the latter who recently challenged our interest?first incidentally, then temporarily, and finally permanently. ...
متن کاملImpact of bronchodilator use on the prevalence of COPD in population-based samples.
The aim of this study was to describe the impact of using bronchodilators on the prevalence of Chronic Obstructive Pulmonary Disease in a population-based survey (Platino study). A cluster sampling of subjects 40 years of age or older, representative of the metropolitan areas of 5 Latin American cities (Sao Paulo, Mexico, Montevideo, Santiago and Caracas) was chosen. Spirometry according to ATS...
متن کاملBronchodilator Response in Patients with Persistent Allergic Asthma Could Not Predict Airway Hyperresponsiveness
: Anticholinergics, or specific antimuscarinic agents, by inhibition of muscarinic receptors cause bronchodilatation, which might correlate with activation of these receptors by the muscarinic agonist methacholine. The aim of this study was to determine whether a positive bronchodilator response to the anticholinergic ipratropium bromide could predict airway hyperresponsiveness in patients with...
متن کاملChange in forced expiratory time and spirometric performance during a single pulmonary function testing session.
BACKGROUND Among patients with obstructive lung disease, the correlation between clinical improvement and bronchodilator response is poor. Forced expiratory time (FET) may explain some discrepancy, but FET has received little attention. METHODS We analyzed change in FET during the 3 initial satisfactory flow-volume loops in 102 consecutive patients, 37 with normal spirometry and 65 with airfl...
